OPEN ENROLLMENT! For children grades Pre-K through High School. Register your child for a fun learning experience of Qur'an and Islamic Studies every Sunday. Classes from September to June. Send email to akher@isocmasjid.org for any inquiries.
By submitting this form, you agree that you are the parent or legal guardian of the child(s) who I stated in this form, and that he/she is under my care and responsibility. I hereby consent and permit my child to participate in outdoor play and activities including but not limited to sports, dramatic play, and entertainment organized by Insight Academy. I also hereby give permission for my child to receive emergency medical treatment in the event that my child needs it.
