This form is to sign up to volunteer as a driver for the Pick & Drop service for Jummah Prayers at MNN Mosque. Once you have filled out this form, someone from our team will get in touch with you to connect you with a senior member of our community. By submitting the form below, you agree to volunteer for the Pick & Drop program and adhere to program guidelines and safety protocols. Your information will be kept confidential and used solely for program-related purposes. MNN WAIVER, RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT I agree that by enrolling as a volunteer in the Pick & Drop program: I understand that MNN Canada is simply responsible for connecting me with a senior to assist them with pick & drop services to the mosque. I take full responsibility for any interactions between myself and the senior. I do voluntarily elect to accept and solely assume all risks of injury incurred or suffered while participating in this program. In addition to giving my full consent for participation, I do hereby waive, release, discharge and agree not to sue MNN Canada, and their elected officials, employees and volunteers or any person or entity connected with them for any claims, damages, costs including attorney’s fees, or cause of action which I may have in the future as a result of damages, injuries, including death, sustained or incurred by my participation from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released. I agree that I shall hold harmless and fully indemnify the parties hereby released from any and all claims, damages, costs including attorney fees, and causes of action which may arise from any cause of action made by me, even if the damages, injuries or death are caused in whole or in part by any of the persons or entities hereby released. BY SUBMITTING THIS FORM, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND EACH AND EVERY ONE OF THE ABOVE PROVISIONS IN THIS WAIVER, RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT AND AGREE TO ABIDE BY THEM.
